No Cost to Facility
Beulah Family Clinic is a certified medical service provider. Our provider services are billed to a wide range of medical insurance. For Medicare or the HMO, provider services are covered by Medicare Part B, section 4432 (b} of the Balanced Budget Act of 1997 (BBA, P.L. 105-33). The original list of exclusions is in section 1888(e) (2) (A) of the Act. We also accept private pay clients.

Beulah Family Clinic providers use state-of-the-art electronic medical documentation software to maintain/standardize records and ICD10-compliant diagnosis criteria.
Meeting State & Federal Rules
Beulah Family Clinic providers are well-trained in regulatory guidelines and provide services that meet and exceed all applicable regulatory guidelines. During facility surveys, our providers are available to assist and support the facility staff.
Appointments
Beulah Family Clinic wants to provide our patients with excellent customer service, and we value your time. To ensure everyone is seen promptly, we ask that you schedule an appointment by calling 682-405.1934 or through our website. A drive-through Clinic is also available. You can schedule an appointment after work or after school. Concierge visits are available to our corporate clients. If the office becomes busy, you will be asked to leave a phone number where you can be reached.
